CDL drivers need to self-certify with IDOT by Thursday
Every commercial driver?s license holder is required to self-certify to the type of driving they do, interstate or intrastate, and if applicable, submit a copy of their Medical Examiner?s Certificate to the Iowa Department of Transportation.

Every commercial driver?s license holder is required to self-certify to the type of driving they do, interstate or intrastate, and if applicable, submit a copy of their Medical Examiner?s Certificate to the Iowa Department of Transportation.
The federal rule requires all CDL holders to be self- certified by Thursday regardless of CDL expiration date.
If not, IDOT must mark the driver record as not medically certified and initiate a downgrade of the CDL to a noncommercial status.
For CDL holders who have not yet self-certified, it can be done online at mymvd.iowadot.gov.
Enter name, date of birth, last five digits of social security number and Iowa driver?s license number.
Select the ?Self-Certification and Medical Examiner?s Certificate? application.
Select the type of commercial motor vehicle operation driven or expect to drive. Based on selection, users may be required to provide a copy of a valid Medical Examiner?s Certificate. If a certificate is required, instructions will be provided for emailing, faxing or mailing the forms to IDOT.
Special note for drivers who self-certify to ?non-excepted interstate:? Since it is not possible to upload a Medical Examiner?s Certificate, the on-line service provides instructions to download the self-certification form and submit it along with a Medical Examiner?s Certificate to IDOT via one of the following:
? Regular mail: Office of Driver Services, P.O. Box 9204, Des Moines 50306.
? Email a scanned copy to: CDLMedCert@dot.iowa.gov
? Fax to: 515-239-1837.
? Present the certificate in person at a local driver?s license station.

For those without online access, go to an Iowa driver?s license issuance location before Thursday. The clerk will ask the type of commercial motor vehicle operation. If it falls into a certain category, a Medical Examiner?s Certificate is needed so it can be scanned and saved with the driver record.
Until the federal program is fully implemented on Jan. 30, 2015, a driver must still carry an original or copy of his or her Medical Examiner?s Certificate, and provide a copy to his or her employer for the individual?s driver qualification file.
