This week, Walmart and Henry County Health Center (HCHC) Foundation celebrated a $25,000 grant award to help fund the Healthy Henry County Communities? (HHCC) Care Links and Connections. The project will facilitate transportation to and from medical provider services and improve care transitions from acute care to post-acute settings.
HHCC will use funds to expand access to care, develop a coordinated continuum of care and improve the quality of essential health care services. Care Links and Connections will partner with existing public health, SEIBUS, HHCC and HCHC staff to coordinate and monitor patient activities between community resources.
In response to local needs of area communities and the changing environment of health care, the Walmart Foundation grant award will be used to improve access to care and support services for the residents of Henry County. Specifically funds will help expand the days and hours of operation for medical transportation services throughout the county, ease access to medical care for residents and help medical providers and patients better coordinate transitions from acute to post-acute care settings. By working with community partners it is the intent of the project to ensure efficiency and reduce duplication of services resulting in a healthier population.
?HCHC Foundation was very glad to facilitate this grant award for Healthy Henry County Communities. The Care Links and Connections Program will be a critical part of the HHCC network and their community wellness efforts,? said HCHC Foundation Director, Michelle Rosell.
Rosell went on to say, ?The HCHC Foundation is particularly excited about the enhancement of county-wide medical transportation services given the growing number of elderly and medically needy in our area.?
The HCHC Foundation project was one of only 14 grants across Iowa selected for funding by Walmart and the Walmart Foundation. Through its State Giving program, the Walmart Foundation presented grants to nonprofits dedicated to serving residents through its core areas of giving, including hunger relief and healthy eating, sustainability, career opportunity and community engagement.
To be considered for support, perspective grantee organizations must submit applications through the Walmart Foundation State Giving Program?s online grant application. Applicants must have a current 501(c)(3) tax-exempt status in order to meet the program?s minimum eligibility criteria.
