WCHC adds pharmacy specialist

WASHINGTON - Washington County Hospital and Clinics is pleased to announce Sarah FitzPatrick, Pharm.D., BCACP has officially begun seeing patients in her role as Clinical Pharmacy Specialist in the WCHC Community Health and Wellness Department as of November 2023.
“We are very excited to welcome Sarah FitzPatrick to her new role as a Clinical Pharmacy Specialist at WCHC,” said Todd Patterson, Chief Executive Officer at WCHC. “Sarah’s work with patients and providers will help ensure that prescribed medications are optimally meeting patients’ needs and goals of care.”
FitzPatrick received her Doctor of Pharmacy degree from the University of Iowa College of Pharmacy in Iowa City in 2021. She then completed two years of postgraduate residency training specializing in Ambulatory Care Pharmacy at the University of Iowa Hospitals & Clinics. Ambulatory care pharmacists practice in outpatient care settings to provide education and collaborative management of chronic disease states together with patients and their providers. Formerly Sarah Greiner, she was an employee of WCHC Beans Pharmacy for 10 years prior to her transition to this new role. Sarah is board-certified in ambulatory care pharmacy through the Board of Pharmacy Specialties and is a member of the Rho Chi Pharmacy Honor Society and the Phi Lambda Sigma Pharmacy Leadership Society. She also teaches on a volunteer basis as an Adjunct Assistant Professor in the Department of Pharmacy Practice and Science at the University of Iowa College of Pharmacy.
As a lifelong resident of Washington, Sarah is proud to serve her local community and bring clinical pharmacy services to our rural health system.
Sarah will work with our team of medical providers to provide collaborative management of medications used for chronic disease states such as anticoagulation therapy and others. Patients may also self-refer to Sarah for assistance with complex medication regimens. She will practice in the WCHC Community Health and Wellness department, located adjacent to the Ken and Rosemary Hanson Specialty Clinic at 400 East Polk Street, Entrance 8 in Washington.
