Howard Cook, 64, of Fairfield, died Thursday, Nov. 29, 2007, at University of Iowa Hospitals and Clinics in Iowa City.
A service will be at 2 p.m. Monday at Raymond Funeral Home in Fairfield, with the Rev. Richard White officiating.
Visitation will be from 3-4 p.m. Sunday at the funeral home.
