Dennis Wade Phillips, 62, of West Burlington, died on Sunday, May 2, 2004, at his home.      The funeral service will be at 1:30 p.m. on Thursday, May 6 at Lunning Chapel in Burlington, with the Rev. Richard W. "Dick" Allen, 77, of Mt. Pleasant, died on Sunday, May 2, 2004, at the Henry County Health Center in Mt.
