Pleasant, died Sunday, July 11 at Iowa Methodist Medical Center in Des Moines.      A memorial service will be at 2 p.m. Saturday, Aug. 21 in the PEO Memorial Rooms at Iowa Wesleyan College. Blaine "Slim" Mark Applegarth, 88, of Burlington, died at 10:38 p.m., Saturday, July 9, 2004, at the Burlington Care Center.      The funeral service will be at 10 a.m. Tuesday, July 13, at Burlington Apostolic Christian Church, with ministers of the church officiating. WATERLOO-Amy A. Allen, 36, of Des Moines, died Monday, July 12, 2004, at home, of congenital heart failure.      Funeral services were held Friday in Waterloo. Burial was in Mt.
