The Amish Cook, Elizabeth Coblentz, 66, of rural Indiana, died Tuesday, Sept. 17, 2002, after collapsing in Blue Springs, Mo., while on a book-signing tour. PHOENIX, Ariz. - Thomas J. Skubal, 69, of Phoenix, Ariz., died Friday, Sept. 13, in Phoenix, Ariz.      In accordance with Mr. Skubal's wishes, there will be no funeral services. His final wish was to be cremated. His ashes will be taken to Iowa, Oct. 27, and scattered over the Mississippi River. A memorial gathering is planned from 2 to 4 p.m. Oct. 27, at the Masonic Lodge in Columbus City.
