Jefferson County Health Center welcomes Amy Martin, expands Comprehensive Diabetes Care Services

Jefferson County Health Center (JCHC) is proud to welcome Amy Martin, RN, CDCES, as its new Diabetes Care & Education Specialist. Amy brings a wealth of experience and a familiar, trusted presence to the community. Her role focuses on personalized, one-on-one diabetes education and support — empowering patients with tools and strategies to manage their condition and improve overall health.
JCHC’s Diabetes Education Program is certified by both the American Diabetes Association and the State of Iowa, meeting the National Standards for Diabetes Self-Management Education and Support. This recognition allows JCHC to bill most insurances for diabetes education and ensures that patients receive high-quality, evidence-based care.
“I work closely with individuals to help them understand their diabetes, prevent complications, manage medications, monitor glucose, and navigate the emotional aspects of living with a chronic condition,” said Martin. “Every person’s diabetes journey is different, and I’m here to help them problem-solve and find practical, affordable solutions that work for their lives.”
In addition to individual appointments, Amy serves as a resource for health care providers managing complex diabetes cases, helps patients access supplies and medications, and offers guidance on diabetes technologies such as insulin pumps and continuous glucose monitors (CGMs).
Sarah Greiner, Director of Clinics at JCHC, added, “Amy’s expertise is already making a difference for patients. By offering personalized guidance, support, and practical tools, we’re helping people improve their health outcomes — not just manage a diagnosis. This is a big step forward for chronic disease care in our community.”
JCHC also offers Diabetes Group Education Classes every other month, which are ideal for those who are newly diagnosed or have not previously received formal education. These classes cover all nine aspects of diabetes self-management and are taught by a multidisciplinary team including a registered dietitian, pharmacist, physical therapist, and Amy Martin. Insurance is billed, and registration is required.

In addition, the JCHC Diabetes Community Group meets monthly and is open to anyone living with diabetes or supporting a loved one with the condition. These free gatherings provide a supportive space to share experiences, discuss timely topics, and connect with others.
